Important Information About Your Upcoming 22-26 Month CORD-CHD Follow-Up Appointment
We’re excited to connect with you and your baby for a follow-up visit. This will take approximately one hour and will be conducted over a video call at a time and location that works best for you.
What to Expect:
- Duration: About 1 hour, over a video call.
- Location: In your home or any convenient spot for you and your baby.
- Technology: We’ll use Zoom for the video call. The study coordinator send you the link and join the call to assist you.
Before Your Visit:
- Make sure the time you pick is not to close to nap time or to a meal, so that your baby is well rested and is not hungry.
- Find a quiet, well-lit space for the call.
- It’s best to have two people available—one to manage the phone or tablet, and one to hold the baby.
- Headphones can help you hear clearly without distracting your baby. If your child hears the examiner, it can sometimes cause distractions.
- If possible, sit on the floor with a blanket or on a large couch.
Visit Structure:
Part 1 – Examining Your Baby
- You will help the examiner by moving your baby as directed and sharing your observations.
- You may need a shaker/rattle and a favorite toy for your baby.
- The examiner will show you short video clips to demonstrate each movement before you perform it.
Part 2 – Answering Questions About Your Child
- We’ll ask about your baby’s skills and may ask you to show certain activities.
- Your baby can play nearby while we chat. We’ll guide you through this process.
- Please have the following toys and items ready:
Items to Have Ready:
- 2 preferred toys (including one that requires action, like pressing a button, spinning, or winding up)
- Board book
- Crayon and paper
- Small snacks (e.g., Cheerios, puffs, etc.)
- Small plastic bottle or similar container
- Washcloth or small blanket
- Toy car or truck
- Common objects for labeling and play: ball, cup, spoon, stuffed animal
- A few small blocks
- Small bowl or plastic cup for placing objects in and on
- Snacks and water in case your child needs a small break
We look forward to seeing you and your baby during the visit! If you have any questions or need help preparing, please don’t hesitate to reach out to the coordinator at your site.
